Community outreach services unit application

You can use this form to apply for case management and outreach supports for housing and community outreach services. This applies to you if you: are homeless, or have a history of homelessness and/or housing instability; have complex mental, physical or cognitive health need, and/or addiction challenges; need outreach supports; or need ongoing outreach supports to maintain a successful tenancy and independence in the community.
