Important instructions to complete this form

• The full names and addresses of directors are required to be recorded and displayed in the societies’ public registry as from April 1, 2021.
• Only 1 address per active director or officer will be accepted and you can use one of any of the following options:
  • residential address;
  • mailing address; or
  • another address at which records can usually be delivered to the director between the hours of 9 a.m. and 4 p.m. from Monday to Friday
