Community outreach services unit application

You can use this form to apply for intensive case management and outreach supports. Use this form if you are experiencing:

• complex mental or physical health needs;
• substance use challenges;
• housing instability or homelessness;
• food insecurity; or
• need assistance navigating various support systems, and need outreach supports to increase independence in the community.

Community outreach services are voluntary. If applying for someone other than yourself, the individual must give consent to do so.
