The Centre de Santé Constellation Health Centre will start seeing clients on Monday, November 7, at the Nuvo Building at 102 – 4149 4th Avenue in Whitehorse. Constellation will be the first primary health care clinic of its kind in the Yukon and serve as a model of care to build upon in the future.
462 applications for 693 Yukoners have been received as of November 1. Applications continue to be reviewed and prioritized by a team of professionals at the clinic. Yukoners not initially accepted will be added to the waitlist and notified when space becomes available.
This ongoing rolling intake will continue over the coming months as Constellation works to recruit primary health care providers and eventually move into its permanent space at 9010 Quartz Road. This phased approach will ensure an equitable and balanced client onboarding and that all clients accepted will receive high-quality care.
Any Yukoner over 16 years of age with a valid Yukon Health Care Insurance Plan is eligible to apply to become a client of the clinic. Individuals younger than 16 years of age may have a guardian apply on their behalf. Yukoners on the Find a Family Doctor waitlist who are interested in receiving care at the Constellation Health Centre must also apply.

We are committed to improving care for all Yukoners and expanding access to primary health care services for the Yukon’s francophone population. The Centre de Santé Constellation Health Centre is the first primary care clinic of its kind in the Yukon and another step in our work to transform our healthcare system into a national leader through the implementation of the Putting People First recommendations. The ramp-up of the clinic over the next few months will ensure that even more Yukoners will be able to access high quality primary care services.

We are pleased with the opening of the first bilingual Centre de Santé Constellation Health Centre in the Yukon. Health is a priority for our growing community. We would like to thank the Yukon government for this initiative and the Partenariat communauté en santé (PCS) for its major contribution in this project for over 20 years. These collaborations allow us to respond to a growing demand for French-language services in the area of health.

At launch, the collaborative care team will consist of nurse practitioners, a registered nurse, a licensed practical nurse and a social worker. Most speak both French and English.
All care will be provided at the clinic and some virtual options will be available.
As a new program, Constellation is being rolled out in a phased approach. It will open in an interim space at 102 – 4149 4th Avenue while renovations continue at the permanent clinic location (9010 Quartz Road, beside the Whitehorse Health Centre), which is expected to open in 2023.
The new collaborative care clinic is a recommendation of the Putting People First report.
