Statement from Minister Tracy-Anne McPhee on Yukon Coroner’s Inquest Report

Minister of Health and Social Services Tracy-Anne McPhee has issued the following statement:

“The Yukon coroner’s inquest into the death of Kaiya Stone-Kirk has been completed. I extend my sincere thanks and appreciation to the presiding coroner and the jurors for their thorough examination and recommendations that were shared with the Government of Yukon in their final report.

“The inquest process is not intended to find fault with individuals or place blame, rather it is a valuable way to find gaps and much needed opportunities for improvements in systems, policies and processes.

“The 13 recommendations in the final report represent comprehensive suggestions towards continually improving the safety of infants and children across the Yukon. Our government is working to respond to each recommendation and improve the safety of children receiving services from Family and Children Services.

“We are committed to working closely across Yukon government departments and with Yukon First Nations governments to make the changes needed to help keep children safe, protected and cared for.”
