Use this form as a bill of sale when you sell your vehicle.

You can also use it as a proof of ownership for your vehicle. This is useful to:

• get a weigh station exemption permit;
• apply for a shipping rebate for a used zero-emission vehicle;
• apply for a rebate for a new light duty zero-emission vehicle; or
• apply for a rebate for a new medium or heavy duty zero-emission commercial vehicle.
