Yukon cancer screening notification opt-out form

Use this form if you do not want to receive invitations, reminders, or other notifications from Yukon cancer screening programs.

You can choose to opt out of notifications from:

• ColonCheck Yukon;
• the Cervix Screening Program;
• Yukon Mammography Program breast cancer screening invitations; or
• all current and future Yukon cancer screening programs. 

You can still access cancer screening services after you opt out. You will be responsible for keeping track of when you need screening, with support from your health care provider.

Submit the completed form

You can submit the form:

• by mail: ColonCheck Yukon, 305 Jarvis Street, Whitehorse, Yukon Y1A 2H3; or
• electronically: email [email protected] to request a secure submission option.

For your privacy, do not send the completed form by regular email.

Your request may take up to 30 days to process. You may continue to receive notifications during this time. Your opt-out will remain in place until you ask to reverse it.
