Breast cancer self-referral form

Use this form to request a routine screening mammogram through the Yukon Mammography Program without a doctor’s referral. Eligible Yukon residents aged 40 and older who have no breast cancer symptoms can complete and submit it directly to Whitehorse General Hospital by email, mail, or fax.
