Order a certificate of change of name

A certificate of change of name is an official document issued under the Change of Name Act. It confirms:

  • your previous name;
  • your new legal name; and
  • the date your name changed.

You may need this certificate to prove your legal change of name when updating:

  • identification;
  • financial records; or
  • other official documents.

You receive 1 certificate of change of name at no cost when your application is approved.

You can order a replacement certificate if you’ve lost your original certificate.

Who can apply

Anyone who has knowledge of the person to whom the certificate relates can apply for a certificate of change of name. You demonstrate this knowledge through the information you provide on the application.

  1. Complete the application for certificate or search for a change of name.
  2. Submit your application and payment:
    • by mail; or
    • in person.

Fees

A certificate of change of name costs $10.

We accept:

  • cash;
  • debit;
  • Visa;
  • Mastercard;
  • American Express; and
  • money orders.

If you apply for a certificate or search and no record is found, we retain the $10 fee to cover administrative costs.

Processing times

Processing can take up to 10 business days.

Receive your certificate

When you apply, choose whether you want to:

  • pick up your certificate in person; or
  • receive it by registered mail.

If you choose to pick up your certificate, we’ll call you when it’s ready.

If you choose registered mail, we’ll mail the certificate to the address provided on your application.
