Access or update your vaccine record

You can use secure online services to submit, update, view or download your vaccine record. This make it easier to:

  • share information with the Government of Yukon; and
  • access personal vaccine records when needed.

Ensuring your vaccine record is up-to-date helps:

  • your health care team provide the best possible care;
  • prevent unnecessary vaccinations; and
  • protect you and your community from vaccine-preventable diseases.

Your personal health information is collected and stored securely under the Health Information Privacy and Management Act.

Submit or update your vaccine record

Use the Citizen Submission Portal to securely add or update your vaccine record in the Yukon Immunization Registry.

To access this service, you must:

  • have a valid Yukon Health Card; and
  • provide your record in English or French.

You can also visit your local health centre to update your vaccine record.

Translate your vaccine record

Your record must be in English or French to use this online service.

If your record is in another language, you must submit:

  • the original record; and
  • a translated copy of the record.

Translate your vaccine record

After you submit your record

  1. Save a copy of your submission confirmation and number.
  2. A health care provider will review your vaccination record and update your health records.

    It may take 10 to 14 days to process your submission. We’ll contact you directly if any of your information:

    • is missing; or
    • requires further verification.
  3. Once we update your record, you’ll be able to access and download your vaccination record through the vaccine record online service.

View your vaccine record online

Use this online service to view, download or print part of your vaccine record. To use this service, you must have a valid Yukon Health Card. 

If you are using a mobile device, access this service using your Chrome, Firefox or Safari browser to avoid issues downloading your immunization record PDF.

Your online record updates weekly.

This online service shows your: 

  • 2 most recent flu vaccines; 
  • 3 most recent COVID-19 vaccines; and 
  • other vaccines. 

Contact your local health centre to request: 

  • your full vaccine history as documented in the Yukon Immunization Registry, including all of your COVID-19 and influenza vaccines; or 
  • records for a dependent. You can only access your own record through this online service.
