How to provide private care services in long-term care homes

If you’re a privately hired health care provider offering services in a long-term care (LTC) home, you must follow these guidelines.

This helps:

  • protect clients’ health information;
  • maintain a high level of care; and
  • prepare LTC staff for your visit. 

Before you provide services in a LTC home

Get consent to share health information

The client or their substitute decision maker must sign a release of health information form. This lets you and LTC staff share health information. 

Contact the LTC staff

Decide with the client or the person booking your visit who will tell the LTC staff about your appointment.

When you arrive  

Check in at the nursing station

Go directly to the nursing station when you enter the LTC. Before you provide care, you must check in with LTC nursing staff and tell them who you’re providing care to.

After your visit

Document the care you provide

Provide a completed progress note to the LTC nursing staff. You must include:
•    a short summary; and
•    any follow-up needs or recommendations.
