This territory-wide service works closely with other government departments, First Nations governments, medical facilities and community partners.  

The program provides:

  • acute;
  • chronic;
  • palliative;
  • rehabilitation services; and
  • respite care.

Services are based on an individual’s assessed needs.

Services in Whitehorse

Whitehorse services include:

  • care coordination;
  • social work;
  • home support;
  • occupational therapy;
  • physiotherapy;
  • nursing; and
  • speech-language pathology.

Services in communities

Community services include:

  • care coordination;
  • regional home support workers;
  • community-based nursing; and 
  • regional therapy services (physiotherapy, occupational therapy or speech-language pathology).

Eligibility to access this service

Yukoners of all ages are eligible if they:

  • have an assessed need for home care; and
  • have, or have applied for, the Yukon Health Care Insurance Plan.

  3. Home care staff will fill out a referral form with you over the phone.
  4. Home care staff will arrange a visit to discuss your referral and assess your needs.
