These programs help you learn how to manage your chronic health conditions. If you were recently diagnosed or want to improve your health, these programs will help.

Getting started: What is pre-diabetes

A registered nurse leads this program and will help you understand:

  • pre-diabetes;
  • diabetes;
  • risk factors;
  • diagnosis; and
  • the important management strategies for a healthy life.

Diabetes Wellness Program

This is a 2-part workshop where you will:

  • connect with others living with diabetes;
  • take practical steps to eat healthy, manage medication, and stay active; and
  • learn from a variety of diabetes specialist such as:
    • dietitians;
    • nurses;
    • pharmacists; and
    • exercise specialists.

Diabetes management- Insulin 101

This is a 2-hour session to help you understand insulin. We recommend this program for people who:

  • already know about diabetes; or
  • have attended the Diabetes Wellness Program.

In this session, you’ll:

  • get expert guidance on insulin use;
  • connect with peers; and
  • increase your confidence in insulin use.

Next level ‘Carb Counting’ for diabetes management

A registered dietitian and certified diabetes educator lead this workshop. It focuses on managing carbohydrates intake for better blood sugar control.

We recommend this program for people who:

  • already know about diabetes; or
  • have attended the Diabetes Wellness Program.

Your health your way

This is a 6-week online program. We created this program in partnership with Self-Management British Columbia. It aims to help people who have ongoing or are at risk of developing chronic condition:

  • better manage your symptoms;
  • learn self-management skills; and
  • reach goals by taking small steps and discover tips to manage day-to-day activities.

You’ll also get support from people with similar health concerns.

Making sense of pain

A self-management program developed in collaboration with PainBC. We designed this program for people living with chronic pain. This 10-week workshop will provide participants opportunities to learn about pain through:

  • the various factors that can cause or make pain worse; and
  • simple strategies that can help participants better manage pain in everyday life.

How to join the programs

Eligibility criteria

  • Any resident of Yukon who has a chronic health condition can register for any of these programs.
  • Anyone interested in improving their overall health is also eligible.
